预期用途】
本试剂盒可用于半定量检测人血液样本中CRP的含量,是鉴别诊断细菌性感染和病毒性感染感冒分型的有效手段之一,可广泛应用于临床医疗单位、社区医院、检测中心或个体患者。
CRP作为一种敏感的炎症标志物,在感染发生后6~8h即开始升高,24~48h达到高峰,高峰值可达正常的数百倍,在感染消除后其含量急骤下降,一周内可恢复正常。而CRP在病毒感染时无显著升高,这为疾病早期感染类型的鉴别提供了极其重要的依据,因此,CRP的检测可以作为细菌性感染和病毒性感染的鉴别诊断。

检验原理】
本试剂盒采用双抗体夹心及胶体金免疫层析技术制作。用一株CRP单抗和羊抗鼠IgG包被NC膜,另一株CRP单抗标记胶体金。检测时样本中的CRP抗原与胶体金标记的CRP单抗结合,形成免疫复合物,该复合物在层析作用下,沿NC膜向前移动,在包被有CRP单抗处(检测线,T线)形成胶体金标记CRP单抗~CRP抗原~包被CRP单抗的夹心免疫复合物,并呈现紫红色,样本中被检物质的多少与检测线显色强度成正相关,在规定检测时间内通过对比试剂盒中配备的标准比色卡色标线强度便可半定量判定检测样本中CRP的含量;过剩的胶体金标记物继续沿NC膜层析至包被有羊抗鼠IgG的质控线(C线)处,产生羊抗鼠IgG与标记胶体金的抗体结合呈现紫红色,表示检测结果有效。

样本要求】
1.全血样本:
1.1指尖末梢血:采血环境温度为室温(10-30℃),样本稀释液平衡到室温使用。先用试剂盒中配备的酒精(消毒)片对待取样本指尖进行消毒,又用试剂盒中配备的一次性采血针轻扎消毒后的指尖,再用试剂盒中配备的一次性微量取样管迅速取10μl全血样本插入试剂盒中配备的样本稀释液中,上下摇动,充分混合均匀,待用(见下图)。
1.2静脉血:按临床检验常规方法采集,用移液器取10μl全血样本注入试剂盒中配备的样本稀释液中,上下摇动,充分混合均匀,待用。
2.血清、血浆样本:按临床检验常规方法采集,采集后立即分离出血清、血浆用于检测;如样本中有絮状物或浑浊有沉淀,应以3000r/min离心10 min后取上清样本5μl注入试剂盒中配备的样本稀释液中,上下摇动,充分混合均匀,待用。
3.采集样本时避免样本溶血,高血脂、高黄胆和溶血样本不能用于检测。
4.采集后的样本应尽快进行检测,如血清、血浆样本不能马上检测,可储存于2~8℃,并在48小时内完成检测;需要长期储存的样本,可于-20℃以下条件保存。
5.冷藏保存的样本,检测前须取出平衡至室温,如样本中有絮状物或浑浊有沉淀,应以3000r/min离心10 min后取上清样本检测。
6.冷冻保存的样本避免反复冻融。

1. 全血
1.1指尖末梢血:
1.1.1撕开铝箔袋,取出检测卡平放于桌面;
1.1.2取下采血器盖子,将稀释好样本的样本管滴头垂直对准样本加样孔,慢慢滴加2滴样本于加样孔中(待第1滴样本完全吸收后再滴入第2滴);
1.2静脉血:
1.2.1撕开铝箔袋,取出检测卡平放于桌面;
1.2.2用移液器取60~70μl稀释后的样本慢慢滴加至样本孔中(或用铝箔袋中滴管加入2滴,待第1滴样本完全吸收后再滴入第2滴);
1.3开始计时,10分钟后判定检测结果,超过15分钟后的检测结果无效;
1.4取出试剂盒中配备的标准比色卡,在10~15分钟内观察检测线(T)的显色强度,并与标准比色卡色线对比。
2.血清/血浆
2.1撕开铝箔袋,取出检测卡平放于桌面;
2.2用移液器取60~70μl稀释后的样本慢慢滴加至样本孔中(或用铝箔袋中滴管加入2滴,待第1滴样本完全吸收后再滴入第2滴);
2.3开始计时,10分钟后判定检测结果,超过15分钟后的检测结果无效。
2.4 取出试剂盒中配备的标准比色卡,在10~15分钟内观察检测线(T)的显色强度,并与标准比色卡色线对比。

检测结果的解释】
1. 只出现一条质控线(C线),检测线(T线)不显色,表明检测结果为阴性。
2. 出现一条质控线(C线),检测线(T线)显色强度低于标准比色卡10μg/ml标准色线,说明检测样本中CRP含量低于10μg/ml。
3. 出现一条质控线(C线),检测线(T线)显色强度在10~50μg/ml之间,与标准比色卡相关含量色线对比显色强度,确定半定量结果。
4. 如检测线(T)显色强度超过标准比色卡50μg/ml色线,表明样本中CRP含量已超过50μg/ml。
5. 无效结果:质控线(C线)不出现色带,说明检测失败,须重新检测。

检验方法的局限性】
1. 本试剂盒检测结果不是临床诊断的唯一确认指标或依据,还应结合临床症状及其它检测结果综合判定。
2. 样本中C-反应蛋白抗原会随着样本放置时间的延长和外界温度上升逐渐降解而无法被抗体识别,可能会出现假阴性结果。

3. 特 异 性:用100ng/ml降钙素原(PCT)、1000ng/ml铁蛋白(FER)、100μg/ml 人血清白蛋白(HSA)检测无交叉反应。
4. 精 密 度:用公司精密度质控品平行测定,结果一致,显色度均一。
5. 样本相关干扰:当胆红素≤0.3mg/ml、甘油三酯≤10mg/ml时不会对检测产生干扰影响。

注意事项】
1. 本品为一次性检测产品,操作前应仔细阅读使用说明书,严格按照说明书的操作程序进行检测并遵守安全操作规定。
2.采血针为一次性使用品,切不可重复使用或混用。
3. 检测标本和所有用过的检测物品请勿随意丢弃,应按国家有关环境保护规定妥善处理。
4. 检测试剂从铝箔袋中取出后,请勿将检测试剂置于空气中过久,避免试剂久置空气中引起胶体金受潮而影响检测结果的准确性。
5. 检测前请勿用手触摸检测试剂的白色观察区或不小心滴上样本等液体。
6. 冷藏保存的检测试剂和待检样本,检测前须平衡至室温后方可检测。
7. 使用前如发现包装铝箔袋已破损,检测试剂可能因受潮而失效,故不能用于检测。
8. 试剂盒在有效期内使用,超过有效期的试剂盒请勿使用。
